Educator Card Application

So that we may better serve you, please bring your library card every time you visit the library.

An educator card is a special library card, allowing you to borrow materials for a longer time period than with a standard library card. Educator cards cannot be used to borrow DVDs or new adult fiction. MCDL materials borrowed with the Educator Card have a loan period of 28 days. Material from other libraries is subject to the owning library’s loan period, which may range from 1 to 21 days. Overdue items accrue fines.

To qualify for an Educator Card, a person must be 18 years of age or older, and

  • Provide educational services as a teacher, daycare provider or group leader
  • Present and maintain a current personal library card from Medina County District Library or any other Clevnet library
  • Present an Ohio Driver’s License or State of Ohio ID card with current address

A library card is similar to a credit card; you use the library card to obtain the use of materials and services. You are responsible for any financial costs associated with the late return, damage or loss of library materials. If the card is lost or stolen, report the loss immediately to any MCDL checkout desk to prevent fraudulent use of your library account.

I agree to observe all rules established by the library, and will be responsible for all materials borrowed on my card, and for the use of this library account number on public computers. I also agree to pay any fines or other charges imposed for late return or damage of library materials. I understand the Educator Card may only be used for purposes connected with my work as a teacher, homeschool educator, daycare provider or group leader. All other library transactions will be made using my personal library card.
